To apply, just return this completed form and ensure that we have an up-to-date health record from each applicant listed below. You must complete a 2017 Health Record Form to register for the conference. Instructions for completing your health records will be sent to you after completing this form.

The Teen Conference runs Wednesday, March 15th to Sunday, March 19th, 2017. Applicants must be 14 years of age at the start of the conference.

Code of Conduct
You are required to read and agree to the Code of Conduct below.

The Teen Conference is designed as a learning experience for teens. Its purpose is to inspire and motivate teens as they begin to prepare for their adult lives. Although there are recreation components this is not a recreational program. Participants will spend their days attending workshops and lectures. Participants will be expected to give their full attention to all speakers and participate appropriately.

It is important for participants to understand that the conference is designed for those with an active interest in learning.

The Teen Conference is a learning based program therefore participants must adhere to the following Code of Conduct.

  1. Participants are expected to attend all parts of the planned program. Inform staff if you are not feeling well or have to miss a session for any reason.
  2. Participants must stay in their own cabins after lights out.
  3. Boys only in boy’s cabins and girls only in girl’s cabins.
  4. Language must be appropriate and respectful of others. No swearing.
  5. Participants are not to leave site at any time unless previously worked out with the coordinators.
  6. Participants will not use tobacco, alcohol, drugs (except those directed by doctor and supervised by Trillium nurses) or remain in the immediate area where they are being used.
  7. Criminal law violations (including, without limitations, theft, drug possession, under-age tobacco use or under-age drinking) will NOT be tolerated.
  8. Avoid roughness and damage to room furnishings, equipment, etc. Participants are financially responsible for any damage or misconduct.
  9. Part of the Teen Conference is to encourage interaction among all participants, but not exclusively with another person. Kissing, cuddling and other sexual display of personal affection distract from the group, and are not acceptable behaviour.
